Every day millions of Americans receive first-rate health care that restores and maintains health. Doctors, nurses and other health care workers are dedicated to keeping patients safe and healthy but, despite the best of intentions, errors occur. Today's health care system is very complex and failure of complex delivery systems is usually what leads to errors — not the single action or decision of an individual. Errors can occur in any health delivery setting: hospitals, clinics, doctors offices, nursing homes, pharmacies, and even care being delivered in your own home.
