About Our Practice
Welcome to First Care, a walk-in primary care practice. We provide comprehensive medical care from routine short-term needs, like annual physicals, to acute care issues, like abrasions, contusions and fractures, as well as the management of chronic diseases. We are conveniently located on Decker Blvd.
Stephen R. Izard, MD, PhD, is board certified in internal medicine. He received his bachelor’s degree, master’s degree and doctorate from the University of Mississippi. Dr. Izard received his medical degree from the University of Mississippi and completed his residency at Fitzsimons Army Medical Center in Denver, Colo. After serving for two years at Moncrief Army Hospital at Fort Jackson, Dr. Izard founded First Care and has been its medical director since January 1985.
|Stewart G. Young, MD, attended undergraduate school at Clemson University and medical school at the University of South Carolina School of Medicine. After completing an internship at Richland Memorial Hospital (now Palmetto Health Richland), Dr. Young began working with First Care.|

The forms below may be printed and completed prior to your arrival. Please bring a copy to your visit.
If an appointment needs to be cancelled, please call at least 24 hours before the scheduled appointment time.
Prescriptions and Refills
Please allow 48 hours for prescription refill requests. You may contact our office if you need additional information about prescriptions.
Fees and Insurance
We accept most insurance plans. If you have a question about our fees or your bill, please call our Physician Practice Billing department at 803-296-7306 and we will assist you. Payment is due when services are rendered. For your convenience we accept cash, checks and credit cards. It is your responsibility to fully understand your coverage benefits and to verify that we are listed as approved providers prior to scheduling an appointment with us.
First Care has a physician on call each night and on weekends. Please be assured your care will be in the best of hands.
From Downtown: Bull St. becomes SC-277 N. Merge onto I-20 E toward Florence. Take the US-1/Two Notch Rd exit, EXIT 74. Turn right onto Two Notch Rd/US-1 S. Turn left onto Decker Blvd.
Our office is 0.4 mile past O’Neil Ct.
From Florence: Take I-20 W toward Augusta. Merge onto Two Notch Rd/US-1 S via EXIT 74 toward Columbia. Turn left onto Decker Blvd. Our office is 0.4 mile past O’Neil Ct.
From Charlotte: Merge onto I-77 S via EXIT 1B toward Columbia. Merge onto SC-277 S via EXIT 18 toward I-20 W/Columbia/Augusta. Take the Parklane Rd exit. Turn left onto Parklane Rd.
If you reach SC-277 S you've gone about 0.2 mile too far. Parklane Rd. becomes Decker Blvd. Our office is approximately one mile down on the left.
