Scheduling Appointments

Our patients are generally referred to us by their primary care physicians. We also accept self-referrals and direct referrals from our patients. To arrange for a consultation please call 803-434-8800. 

Office hours: Monday through Thursday, 8:30 a.m. to 4:30 p.m.; Friday, 8:30 a.m. to 12:30 p.m.

You also may call our administrative office at 803-434-8800 with any questions about our practice or to help guide you through the referral process while we are in a state of transition. 

Please bring with you to your appointment:

  • Any pertinent medical records
  • Any radiographs including x-rays, mammograms, CT scans, MRI
  • Photo ID – Driver's License
  • Insurance Card
  • List of all current medications

The forms below may be printed and completed prior to your arrival. Please bring a copy to your visit.

If you need an interpreter or any other special accommodations, please notify us when you make your appointment.

Prescription Refills

For refills, please call the clinical office between normal business hours only.

Appointment Cancellation or Rescheduling

Please call if you need to cancel or reschedule your appointment. A "No Show" fee may be charged if you fail to notify us of cancellations at least 24 hours in advance.

Emergencies

In case of an emergency or after hours our answering services will assist you and direct your call to the on-call surgeon. The answering service can be reached through our administrative office line at 803-434-8800. (Please note that we share services with U.S.C. Department of Surgery.) You may also call 803-434-6000 and ask for the surgical resident on call.
