Flu shot exemption forms for vendors
Palmetto Health is committed to providing a safe and healthy environment for all of our team members, patients and visitors. Our influenza vaccination program is provided to enhance our current infection control plans in order to prevent or minimize the spread of influenza within the Palmetto Health organization. All vendor representatives are required to have a flu shot before entering our facilities.

Vendor representatives may request an exemption from the mandatory vaccine with documented medical contraindications or as an accommodation to an individual’s sincerely held religious belief or practice. Requests should be consistent with prior vaccination history. All requests for exemption will remain confidential. Requests for exemption must be submitted annually.
